Help at Home, a leading home care provider in Indiana, seeks an RN Case Manager to support patients in LaFayette and Logansport with comprehensive in-home nursing care.

The role requires an Indiana RN license and at least one year of clinical nursing experience, with travel across the territory and the possibility of a $5,000 sign-on bonus. Competitive benefits include health coverage and retirement plans.

Qualifications

• Graduate of an accredited nursing school.
• One year of clinical nursing experience.
• Active Indiana RN license.
• Experience with medically fragile clients addressing medical, social and behavioral components of care.

Responsabilités

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and reevaluates nursing needs.
• Initiates the plan of care and necessary revisions.
• Initiates preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes for patient visits and summaries of care conferences.
• Coordinates services and informs personnel of changes in patient condition.
• Counsels patient and family in meeting nursing needs.
• Processes orders and notifies physician of patient needs and changes in condition.
