Job summary

Help at Home is seeking a Registered Nurse Case Manager to support patients in their homes across Lafayette and Logansport, Indiana. The role includes evaluating nursing needs, initiating and revising plans of care, and coordinating services with physicians and families.

A $5,000 sign-on bonus is available. The position requires an Indiana RN license, at least one year of clinical nursing experience, and the ability to travel weekly.

Qualifications

• Graduate of an accredited nursing school.
• Active Indiana RN license.
• Experience with medically fragile clients and coordinating care.
• Excellent communication and ability to work independently in a fast-paced environment.

Responsibilities

• Provide nursing care per the plan of care.
• Conduct initial evaluations and reevaluate patient nursing needs.
• Coordinate services and inform team of changes in condition.
• Document visits and prepare care summaries in a timely manner.
• Travel weekly within assigned territory to support patients.
