CVS Health is seeking a Case Manager, Registered Nurse in Illinois to provide comprehensive care coordination for dual eligible members. You will liaise with members, employers, providers, and healthcare staff to develop and implement care plans for complex cases, while documenting activities and outcomes.

The role requires an active Illinois RN license, 3–5 years of clinical experience, and 2–3 years in care management or discharge planning.

Qualifications

RN license in Illinois (active and unrestricted).
3-5 years clinical practical experience.
2-3 years care management, discharge planning, or home health care coordination (preferred).

Responsibilities

Act as a liaison with members, employers, providers, and healthcare personnel.
Implement and coordinate case management activities for catastrophic cases and chronically ill members.
Interact with members/clients telephonically or in person.
Assess and analyze medical and vocational status of members/clients.
Develop a plan of care to facilitate appropriate condition management.
Communicate with stakeholders regarding member/client care.
Prepare required documentation of case work activities.
Consult with internal multidisciplinary teams to maximize health outcomes.
Provide educational and prevention information for best medical outcomes.
Monitor member/client progress toward desired outcomes.
