Job summary

Minimum qualifications include an RN license, BLS, and 3 years of recent RN experience. Preferred qualifications include a BSN and relevant certifications, with duties focusing on care planning, rounds, and seamless handoffs across care settings.

Qualifications

• Minimum qualifications: Graduate from an accredited school of professional nursing; Current state licensure as a Registered Nurse (RN); Basic Life Support (BLS) Certification through AHA or ARC; Three years of recent RN clinical experience.
• Preferred qualifications: BSN; Certification within one year of eligibility (CTM, ACM, CCM or AHA); Two years of recent experience in utilization review, quality or care management; Knowledge of acute care and accreditation requirements; Discharge planning and/or Quality Improvement experience.

Responsibilities

• Collaborate with inter-professional teams across the continuum of care to facilitate transitional care coordination.
• Engage in patient advocacy, holistic assessment and development of comprehensive transition plans.
• Conduct patient rounds, huddles and committee meetings to ensure continuity of care.
• Lead effective handoffs to next level of care or community and mitigate barriers to transitions of care.
