Job summary

Qualifications

• Three years of recent RN experience required.
• Two years of recent experience in utilization review, quality, or care management preferred.
• Prior case management experience preferred.
• Current state licensure as a Registered Nurse (RN) required.
• Basic Life Support (BLS) certification required.

Responsibilities

• Ensure compliance with accrediting body standards and state regulations.
• Promote interprofessional synchronization during the acute episode.
• Complete effective handoffs to next level of care or community.
• Assess and mitigate risks to patients’ transitions of care.
• Develop individualized transitional care plans with patients and care partners.
• Participate and lead patient rounds, huddles, and committee meetings.
