Job summary

Highpoint Health Sumner in Gallatin, TN, seeks an experienced Registered Nurse to join as Case Manager/Swing Bed Coordinator. You will coordinate admissions for swing beds, monitor patient care, and collaborate with a multidisciplinary team to ensure timely discharge planning.

Qualifications

• RN license to practice in TN required.
• BSN preferred.
• BLS certification to be obtained within 30 days.
• Minimum 2 years of clinical experience preferred, especially in Utilization Review, Quality Improvement and Case Management.

Responsibilities

• Coordinate/market Swing bed admissions for Trousdale Medical Center and Riverview.
• Monitor patient care and ensure high-quality, cost-effective care.
• Work with multidisciplinary teams to ensure discharge planning is timely and appropriate.
• Conduct discharge planning including transfers and community resource coordination.
• Perform chart reviews to identify issues impacting service delivery and outcomes.
