Job summary

Highpoint Health Sumner in Gallatin, TN is seeking an experienced RN Case Manager/Swing Bed Coordinator to coordinate swing bed admissions and ensure high-quality, cost-effective patient care. You will work with the care team on discharge planning and community resource coordination.

The role requires TN RN licensure, BLS within 30 days, and 2+ years in utilization review or case management. This is a full-time daytime position within Lifepoint Health’s network.

Qualifications

• RN licensed to practice in TN.
• BSN preferred.
• BLS certification within 30 days.
• Min. 2 years in Utilization Review, Quality Improvement or Case Management.
• Ability to work with minimal supervision in a high-stress environment.

Responsibilities

• Consult with nursing staff and multidisciplinary team to evaluate patient status and appropriateness of care.
• Monitor patient and family satisfaction; respond to questions from patients and payors.
• Participate in discharge planning and coordinate transfers to facilities and community resources.
• Perform chart reviews to identify issues with service delivery, outcomes, cost and reimbursement.

How you'll contribute

RN - Case Manager/Swing bed Coordinator Responsible for the coordinating/marketing Swing bed admissions for Trousdale Medical Center and Riverview. Monitors care of patients within case groups to ensure high quality and cost-effective care is provided. Works with other hospital personnel to assure appropriate and timely discharge planning.

Reports to: Director of Case Management

FLSA: Exempt

Qualifications and Requirements

• Graduate of a Program in Discipline Required; Graduate from an accredited school of nursing BSN Preferred
• Licenses: Current RN to practice/work in the state of TN
• Certifications: Basic Life Support (BLS) - to be obtained within 30 days
• Minimum Work Experience Two years recent related clinical experience preferred in Utilization Review, Quality Improvement and Case Management preferred.
• Required Skills. Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Essential Functions

• Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status andappropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.
• Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
