HIGHPOINT HEALTH SUMNER is seeking a Registered Nurse (RN) for a full-time Case Manager/Swing Bed Coordinator role in Gallatin, TN. The role focuses on coordinating Swing bed admissions, ensuring high-quality, cost-effective care, and collaborating with hospital staff for timely discharge planning.

Minimum 2 years of related clinical experience is preferred, with TN RN licensure and BLS certification within 30 days. BSN is preferred, and the job reports to the Director of Case Management.

Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Current RN license to practice in TN.
• BLS certification required within 30 days.
• Minimum 2 years of related clinical experience; Utilization Review/Case Management preferred.

Responsibilities

• Consults with nursing staff and multidisciplinary team to evaluate patient status and care appropriateness.
• Monitors patient and family satisfaction; responds to questions from patients, families, and payors.
• Participates in discharge planning including transferring patients to other facilities.
• Performs chart reviews to identify issues with service delivery, patient outcomes and reimbursement.
