Kansas City Hospice & Palliative Care is seeking a compassionate Registered Nurse (RN) Case Manager to join an interdisciplinary team in Kansas City, MO. The role focuses on delivering expert hospice care, educating families, and supporting patients with dignity and compassion.

Requirements include MO/KS RN licensure (or compact), CPR certification, and at least 1 year of hospice bedside experience. Excellent communication and organizational skills are essential, with a valid driver’s license

Qualifications

• Minimum of one year bedside hospice experience preferred.
• Ability to communicate effectively in oral and written formats.
• CPR certification required.

Position Description:

We are seeking a compassionate Registered Nurse (RN) to join our interdisciplinary hospice team in providing high quality, expert care to our seriously ill patients. Our RN Case Manager is a vital part of our hospice team providing comfort, care, support and education to our patients and their loved ones.

The Ideal Candidate Will Possess the Following Requirements:

• Valid, unencumbered Missouri and Kansas RN or compact license to include both states
• Minimum of one year bedside experience in caring for hospice patients is preferred
• Ability to communicate effectively and professionally with others; and to prepare and present information in oral and written formats
• Superior organizational skills with the ability to multitask and meet deadlines; must schedule and manage time effectively
• Valid driver’s license and insurance plus reliable transportation
• CPR certification required
