Responsibilities

• Guiding patients and families through program orientation and discharge planning.
• Coordinating education for patients, families, and caregivers to participate in treatment planning.
• Developing and implementing individualized treatment plans reflecting patient needs.
• Completing psychosocial assessments and discharge planning evaluations.
• Collaborating with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promoting HCA Healthcare’s values in every patient and family interaction.
