Qualifications

Responsibilities

• Guide patients and families through program orientation, explain rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths, needs and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations; document findings and communicate needs across the care team.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless, patient-centered care.
• Promote HCA Healthcare values of compassion, respect and excellence in every interaction.
