Job summary

Heart’n Soul Hospice is seeking an RN Case Manager in Seattle, WA for a full-time position focused on compassionate, high-quality end-of-life care. The role emphasizes comprehensive patient assessment, care coordination with families and a broad care team, and adherence to regulatory standards.

The ideal candidate is a graduate from an accredited nursing program with an active RN license, a valid driver's license, reliable transportation, and embodies the agency's CHOICE values in daily practice.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse license.
• Current Driver's License with reliable transportation and active auto liability insurance.
• Two years of RN experience in a healthcare/clinical setting preferred.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patient needs related to terminal illness and ADLs.
• Develop and implement the individualized plan of care and revise as necessary.
• Coordinate care among patient, caregiver, physicians and other healthcare professionals.
• Attend IDG meetings and participate in multidisciplinary care planning.
• Provide on-call coverage as assigned and ensure timely documentation.
• Ensure compliance with local, state and federal regulations and agency policies.

RN Case Manager

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
