Job summary

Heart’n Soul Hospice is seeking an RN Case Manager to lead comprehensive assessments of patients facing life-limiting illness and to coordinate a personalized care plan. You will collaborate with physicians and a broad care team to ensure compassionate, high-quality end-of-life care and family support.

Responsibilities include on-call coverage, timely documentation, and adherence to regulations. Eligible candidates are RNs with a state license and a desire to serve in a mission-driven hospice

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Two years of RN experience preferred in healthcare/clinical setting

Responsibilities

• Conduct initial and ongoing assessments of patient needs (physical, psychosocial, environmental) and ADLs
• Implement and revise individualized care plans as needed
• Collaborate with patients, families, physicians and other healthcare professionals
• Attend IDG meetings and coordinate patient care planning
• Provide on-call service to patients/families as assigned
• Submit documentation timely and accurately
• Ensure compliance with applicable laws, regulations and policies

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
