UnitedHealth Group in Baltimore, MD, is seeking a Hospital Safe Discharge RN Case Manager to initiate proactive outreach, remove discharge barriers, and ensure smooth patient transitions. The role emphasizes collaboration with facility staff, physicians, and post-acute services to optimize patient flow.

The ideal candidate holds a BSN and has 5+ years of acute care nursing experience with 2+ years in discharge planning, plus a current RN license (Compact preferred).

Qualifications

• BSN required.
• 5+ years of nursing experience in an acute care setting.
• 2+ years of discharge planning experience required.

Responsabilités

• Assess barriers to a safe and efficient discharge process.
• Coordinate and drive an efficient discharge process with stakeholders.
• Demonstrate knowledge of acute inpatient case management and care coordination.
• Manage assigned case load efficiently utilizing time management skills.
• Prepare and lead periodic rounds to discuss case needs and prioritize discharges.
• Utilize electronic platforms and EMR systems for patient care coordination.
• Confer with Medical Director or leadership for complex case reviews.
• Identify and communicate a member's level of risk and participate in planning.
• Collaborate with hospital staff, patients, families, and community services.
• Support hospital staff in facilitating member transitions to post-acute care.
