Job summary

Hospice of the Valley is seeking a Registered Nurse Case Manager to provide and coordinate hospice care for patients living in facilities and at home within the Phoenix metropolitan area.

The role focuses on delivering comprehensive care through the interdisciplinary team, educating patients and families, and ensuring comfort across physical, emotional, and spiritual needs while maintaining high professional standards.

Qualifications

• Graduate of accredited school of nursing.
• Minimum one year of recent experience in medical, surgical or critical care.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Solution-driven, creative problem solving and attention to detail.

Position Profile

Resident Care Team (RCT)RN case managers have the primary responsibility to provide and coordinate hospice care for patients living inResident CareFacilitiesand to work closely with the facility's staff. Theydeliver care and oversee the interdisciplinary hospice team to ensurepatients' physical, emotional and spiritual needs are met. RN case managers educate patients and families about the disease process, help manage and control symptoms and offer support and strength during difficult times.

Minimum Qualifications

• § Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.

• § Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.

Preferred Qualifications

• § Previous hospice or oncology experience.
• § Knowledge of the death/dying and bereavement process.
