Job summary

Hospice of the Valley in Phoenix, AZ, is seeking an RN Home Admissions nurse for the Casa Grande/Eloy territory. This full-time role (36 hrs/week, day shift) provides initial nursing intake assessments and helps patients transition into our home-based programs.

You will develop relationships with facility and community referral sources, ensure compassionate, one-to-one care, and collaborate with a dedicated team in a supportive, mission-driven not-for-profit environment.

Qualifications

  • Graduate of accredited nursing program.
  • Active RN license required.
  • Minimum 1 year of medical/surgical or critical care experience.
  • Excellent communication and interpersonal skills.
  • Ability to manage time and priorities.

Responsibilities

  • Facilitate admission of patients from health care settings into Hospice of the Valley programs.
  • Provide initial nursing intake assessments in patients' homes or facilities.
  • Develop relationships with facility and community referral sources.
  • Promote exemplary customer service and smooth transitions into programs.
  • Plan care and intervene effectively for admitted patients.
  • Provide patient/family/caregiver teaching and support.
  • Collaborate with the care team to coordinate services.
  • Maintain accurate and timely documentation.

Position Profile

The Admission nurse provides initial nursing intake assessments in various settings to facilitate a smooth transition into Hospice of the Valley programs. The Admission Nurse also develops relationships with facility and community referral sources and promotes exemplary customer service. Care delivery primarily takes place in the patient's home or in a facility where the patient receives care.

Responsibilities
  • Facilitates smooth transition and admission of patients from health care settings into HOV programs.
  • Promotes HOV services and programs.
  • Develops and maintains therapeutic relationships.
  • Plans care and intervenes effectively.
  • Provides effective patient/family/caregiver teaching.
  • Supports the death/dying process.
  • Collaborates in providing patient care.
  • Creates timely and accurate documentation.
  • Utilizes resources effectively and efficiently.
  • Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services.
  • Maintains and enhances professional skills.
  • Adheres to high standards of personal and professional conduct.
Minimum Qualifications
  • Graduate of accredited school of nursing.
  • Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
  • Strong clinical assessment skills.
  • Excellent communication and interpersonal skills.
  • Solution driven, creative and resourceful problem solving skills.
  • Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.
Preferred Qualifications
  • Previous hospice or oncology experience.
  • Knowledge of the death/dying and bereavement process.
