Moments Hospice is seeking an RN Case Manager to deliver compassionate in-home nursing care and care coordination. You will work closely with the owners and support a patient-centered approach in a flexible, entrepreneurial environment.

Responsibilities include client assessments, care plan refinement, and collaboration with the healthcare team to ensure high-quality outcomes. A valid RN license and EMR proficiency are required, along with a reliable vehicle and insurance.

Qualifications

• Licensed Registered Nurse (RN) in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.
