Job summary

moments Hospice in Wisconsin is seeking an RN Case Manager to provide compassionate, patient-centered nursing care throughout the end-of-life journey. You will assess needs, refine care plans, coordinate with families and the healthcare team, and support patients in their final days and weeks.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
