Job summary

Moments Hospice is seeking an RN Case Manager to support patients in their final days, weeks, and months. You will work closely with the owners and contribute to entrepreneurial growth within the organization, delivering high-quality care and advancing clinical practices.

Responsibilities include conducting initial assessments, refining care plans, and coordinating patient care across the clinical team. A valid RN license and reliable vehicle are required for on-the-road work.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.
