Job summary

Tidewell Hospice, part of Empath Health, is seeking a dedicated RN PRN to provide compassionate, high-quality end-of-life care in southwest Florida. You will work under a physician, assess symptoms, manage comfort, and collaborate with the Interdisciplinary Group to update the care plan for patients and families in Manatee and Northern Sarasota counties.

Successful candidates hold a Florida RN license, CPR certification, and ideally hospice experience plus a BSN/MSN.

Qualifications

• Graduate of approved nursing program with an unrestricted Florida RN license.
• Current CPR certification is required.
• Minimum one year of acute-care hospital patient care experience; hospice RN experience preferred.
• CHPN certification strongly encouraged within three years of employment.
• BSN or MSN is preferred.

Responsibilities

• Under the direction of a licensed physician, manages comfort and provides symptom relief using specialized nursing skills for palliative and end-of-life care.
• Assesses impact of terminal diagnosis on physical, functional, psychosocial, and daily living activities.
• Collaborates with Interdisciplinary Group (IDG) to develop and update the plan of care and coordinate referrals.
• Educates patient, family, and caregivers about disease, palliative interventions, caregiving, and safety practices.

Empath Health is currently seeking an RN PRN to join our team.

What You'll Do

• Under the direction of a licensed physician, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial, and environmental needs and activities of daily living.
• In collaboration with the other members of the Interdisciplinary Group (IDG); develops implements and updates the individualized plan of care, initiates appropriate preventive and rehabilitative procedures and initiates referrals to other services; requests complimentary services, as needed.
• Educates patient/resident, family and caregivers regarding disease process and decline, palliative interventions care giving, dying process and safety practices.

What You'll Need
