Job summary

Traditional Home Health Care is seeking an RN Case Manager for pediatric and adult skilled home care in the Dunmore, PA area. This full-time role offers weekly pay, an aggressive bonus plan, and a comprehensive benefits package including medical, dental, vision, 401K and paid time off.

You will supervise a team of RNs/LPNs and Home Health Aides, coordinate referrals, and ensure compliant, high-quality patient care for pediatric and waiver patients.

Qualifications

• Prior experience in scheduling, HR, or customer service.
• Current license as a Registered Nurse in Pennsylvania.
• Strong organizational and time management skills, communication, interpersonal skills, documentation skills, critical thinking, and ability to work independently.
• Ambition to grow and advance with company goals.
• Excellent communication and customer service skills
• Must be able to multi-task in a fast paced environment.

Responsibilities

• Receives referrals, determines services required, and Agency’s ability to meet needs for pediatric and waiver patients.
• Supervisory visits to observe clinical skills and ensure exemplary client care and compliance
• Monitors documentation of pediatric and wavier patient clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness.
• Manages/supervises a team of RNs, LPNs, Home Health Aides to provide effective and quality home care services
