CareGivers America – NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one‑on‑one pediatric care in the home setting throughout parts of East Lackawanna, Wayne, Wyoming, and Susquehanna counties.

This is a salaried Monday–Friday role with a company vehicle provided.

• Develop and oversee individualized pediatric care plans in collaboration with physicians and families
• Provide clinical direction and supervision to LPNs and Home Health Aides

Qualifications

• Active Pennsylvania RN license (Required) – BSN preferred
• 1 year of professional nursing experience preferred
• Pediatric or home health experience preferred
• Strong leadership and communication skills

• Valid driver’s license
• Current CPR certification
