Job summary

Traditional Home Health Care is seeking a Registered Nurse Case Manager for the Dunmore, PA service area (and surrounding Lackawanna County areas). This full-time role runs Monday–Friday 8:30am–5:00pm, offering a $10,000 sign-on bonus and competitive pay with benefits.

You’ll assess patients, develop care plans with the team, administer medications, and ensure timely OASIS documentation, all in home settings.

Qualifications

• Active Pennsylvania RN license required.
• BLS certification required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
• Provide hands-on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation, including OASIS assessments.
• Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care.
• Educate patients and families on treatment plans and self-care techniques.
• Inform patients of additional services and resources as needed.

Your Role As An RN Case Manager
