Traditional Home Health Care in Dunmore, PA, seeks a full-time RN Case Manager to assess patients, develop care plans, and deliver in-home nursing with a patient-centered approach. The role includes coordinating with physicians, therapists, and families to ensure quality, continuous care.

The ideal candidate holds an active PA RN license, BLS, a valid driver’s license, and strong documentation skills; home health experience is preferred. Competitive pay, benefits, and sign-on available.

Responsibilities

• Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
• Provide hands-on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation, including OASIS assessments.
• Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care.
• Educate patients and families on treatment plans and self-care techniques.
• Inform patients of additional services and resources as needed.

Your Role as an RN Case Manager:
