Job summary

The Marion VMAC in Vincennes is seeking a full-time registered nurse to join the CBOC Primary Care Team as an RN Case Manager. You will provide comprehensive, patient‑centered care management for veterans with chronic or high‑risk conditions, perform detailed nursing assessments, develop care plans, and coordinate with an interdisciplinary team.

You will educate patients, support self-management, and ensure adherence to clinical guidelines in outpatient care.

Qualifications

• English language proficiency required for direct patient care.
• Graduate of a nursing program approved by accrediting bodies (ACEN/CCNE).
• Eligible for licensure as a registered nurse in the United States.
• Bridge programs and GNT exception allow temporary appointment pending licensure.

Responsibilities

• Provide comprehensive, patient-centered care management for veterans with chronic or high-risk conditions.
• Conduct detailed nursing assessments, develop care plans, implement evidence-based interventions, and evaluate outcomes.
• Coordinate with interdisciplinary teams and community providers as needed.
• Deliver health education, coaching, and support to enhance self-management.
• Adhere to organizational policies, clinical guidelines, and regulatory standards in outpatient care.
