Job summary

The Marion VA Medical Center in Paducah, KY seeks a full-time registered nurse to join the Primary Care Team as an RN Case Manager. You will deliver patient-centered care, assess Veterans’ needs, develop care plans, and coordinate services across the care continuum to improve outcomes.

Responsibilities include triaging data, collaborating with PACT, HBPC, Mental Health, Social Work and Pharmacy, delivering health education, and promoting self-management.

Responsibilities

• Registered Nurse Case Manager provides patient-centered care management principles to support Veterans in managing chronic, high-risk, or post-acute conditions.
• The RN triages patient-generated data and initiates timely interventions, collaborating with interdisciplinary teams.
• The RN delivers health education, coaching, and motivational counseling to enhance Veterans' self-management skills.
