Job summary

The Marion IL VMAC in Paducah, KY, is seeking a full-time registered nurse to join the Paducah CBOC Primary Care Team. The RN provides comprehensive assessments, develops care plans, coordinates with PACT, HBPC, Social Work, Pharmacy, and Nutrition, and delivers patient education to improve self-management and outcomes.

Active RN licensure in the United States, English proficiency, and adherence to VA qualification standards are required; involvement in interdisciplinary care and patient

Qualifications

English language proficiency and an active RN license in the United States.
Graduate of a school of professional nursing with ACEN/CCNE accreditation or equivalent.
State licensure required; bridge programs acceptable where applicable.
Foreign education must be evaluated for equivalency.
VA qualification standards and grade determinations apply.

Responsibilities

Provide patient-centered nursing care and case management.
Perform comprehensive assessments and develop care plans.
Coordinate with PACT, HBPC, Social Work, Pharmacy and Nutrition.
Deliver health education and motivational coaching to Veterans.
Adhere to VA policies and clinical guidelines; demonstrate leadership.
