Job summary

The Veterans Health Administration, Marion VMAC in Evansville, IN, is seeking a full-time registered nurse to join the Primary Care Nursing Service within the Evansville Community Outpatient Clinic. The role emphasizes direct Veteran care and care coordination within the PACT framework.

The nurse will manage panels, use registries, and promote wellness, disease prevention and chronic care management. Work hours are Monday–Friday, 8:00am to 4:30pm; telework is not available.

Qualifications

• Must be a registered nurse with an active US licensure.
• Licensed in good standing to practice in the United States.
• Mon-Fri 8:00am–4:30pm schedule; telework not available.

Responsibilities

• Provide direct Veteran care and care coordination within the PACT.
• Manage assigned panels and utilize registries to ensure access and evaluation.
• Provide health promotion, health education and coaching on wellness and chronic care management.
• Support nursing education, orientation, competencies, and quality-improvement initiatives.
