Job summary

The Case Management RN manages the transition of care for members post-discharge and those with chronic conditions, coordinating follow-up care and educating patients to prevent hospital readmissions and improve outcomes.

Responsibilities include timely post-discharge outreach, coordinating with care teams, developing individualized plans, and documenting all actions in the EMR.

Qualifications

• RN with active license (RN/LPN or SW)
• Experience in case management, care coordination, or utilization review
• Bilingual in English and Spanish preferred

Responsibilities

• Post-discharge outreach within 24–72 hours to assess needs and confirm instructions.
• Coordinate follow-up appointments with primary care, specialists, and home health services.
• Develop individualized care plans based on program categories and risk factors.
• Educate patients and caregivers using motivational interviewing techniques.
• Document all interactions accurately in the care management platform.
