Job summary

Requirements include NH RN license, a Bachelor's in Nursing (Master's preferred), BCLS within 30 days, CCM/CMC certification preferred, and strong communication skills. Per diem, 8a–5p, with emphasis on efficient care planning.

Qualifications

• : Must hold RN license in NH.
• : Bachelor’s degree in nursing is required; Master’s preferred.
• : BCLS certification required within 30 days of hire.
• : CCM/CMC certification preferred; willing to work toward CCM if not certified.
• : Five years of healthcare experience; case management experience is an asset.
• : Excellent interpersonal and communication skills.
• : Proficiency with Microsoft Office Suite preferred.
• : Ability to manage high-volume requests with attention to detail.

Responsibilities

• : Lead a collaborative process with the healthcare team to coordinate patient care and transitions.
• : Promote patient- and family-centered care coordination and transitions.
• : Connect patients, providers, and community resources seamlessly.
• : Organize patient care activities to optimize resource use.
• : Coordinate social services/case management within discharge planning and across departments.
• : Conduct concurrent medical record reviews and contribute to performance measures.
• : Educate staff on resource utilization, discharge planning, and psychosocial aspects of care.
