Job summary

Homewatch Caregivers of St Cloud Minneapolis St Paul is seeking a dedicated RN Case Manager to coordinate and deliver in-home nursing care. The role includes assessing clients, planning care, and acting as liaison among care teams in client homes and local offices, with occasional remote work as needed.

• Minimum one year of nursing experience (home health or hospice preferred).

• Complete reauthorizations and maintain accurate documentation.
• Provide health education and support to clients and families.

Overview

Seeking a dedicated RN Case Manager to coordinate and deliver in-home nursing care. This role involves managing one client, performing clinical assessments, care planning setup if applicable, and acting as a liaison among care teams. Includes field nursing tasks such as wound care, glucose checks, and medication. This is a casual role with assessments being conducted every 90 days.

Minimum Requirements

• Valid and unrestricted Nurse (RN) license in the State of Minnesota
• BLS certification & CPR (required)
• Oasis experience not mandatory but preferred
• Strong Clinical skills and the ability to conduct comprehensive assessments
• Minimum one year of nursing experience (home health or hospice preferred)
• Exceptional communication and interpersonal abilities

Key Responsibilities
