Job Title: RN Case Manager

BAYADA Home Health Care is seeking an RN Case Manager for our Reading, PA Adult and Pediatric Skilled Nursing office. This office provides top-quality care to adult and pediatric clients throughout Reading, PA and surrounding areas.

Responsibilities of the Clinical Nurse Manager:

• Lead, inspire, and motivate staff to provide exceptional care.
• Conduct assessments of pediatric and adult clients in Reading, PA, and surrounding areas.
• Develop and oversee care plans.
• Supervise LPNs, RNs, and home health aides in the field.
• Facilitate new hire orientation and conduct competency assessments for staff.

Qualifications:

• Registered Nurse (RN) with at least 1 year of experience as an RN.
• Supervisory experience preferred.
