Job Description
This position supports our Complex Adult Behavioral Health Care Management program. Candidates must have a New York LMHC, LMSW, or LCSW license, with experience in adult behavioral healthcare and substance use. Knowledge of BH services/resources and community resources for adults is required. Case management and managed care experience are strongly preferred. Details will be discussed during the interview.
Work hours: Monday - Friday, 8:30am - 5:00pm EST. Remote position; residence in New York required.
Job Duties
- Complete clinical assessments within regulated timelines to determine case management eligibility based on clinical judgment, health changes, psychosocial wellness, and assessment triggers.
- Develop and implement case management plans collaboratively with members, caregivers, physicians, and healthcare professionals to meet member needs and goals.
- Conduct telephonic, face-to-face, or home visits as needed.
- Monitor care plans regularly, evaluate effectiveness, document interventions, and suggest adjustments.
- Maintain an ongoing member caseload for outreach and management.
- Promote service integration, including behavioral health and long-term supports, to enhance care continuity.
- Implement specific Molina wellness programs, e.g., asthma and depression management.
- Facilitate interdisciplinary team meetings and collaboration.
- Use motivational interviewing and clinical guideposts to educate, support, and motivate members.
- Assess barriers to care and provide coordination and assistance.
- Collaborate with RN case managers/supervisors as needed.
- Provide consultation and resources to peers in behavioral health and social sciences.
- Travel up to 40% locally, depending on member needs and regulations.
Qualifications
Required:
- Graduate from an accredited school with a nursing (LVN/LPN) or social science degree (psychology, gerontology, public health, social work, or related).
- Active behavioral health license.
- 2+ years in case management, disease management, or behavioral/medical health settings.
- Experience working with severe mental health and substance use disorders, including foster care.
- Discharge planning experience from various care settings.
- Excellent verbal and written communication skills.
- 1-3 years data entry and 1+ years using clinical platforms.
- Knowledge of behavioral health 'whole person' care principles and managing chronic conditions.
- Proficiency with Microsoft Office (PowerPoint, Excel, Word).
- Experience with clinical documentation software/platforms.
- Valid driver’s license with a good record and reliable transportation.
Preferred:
- Master’s degree in social sciences or related fields.
- Behavioral health therapist credentials (LCSW, APSW, CHEP, LPC/LPCC, LMFT).
- 3+ years in behavioral health case management, managed care, or disease management.
- Field-based or home health experience.
- Certified Case Manager (CCM).
