CareGivers America – NEPA, a Modivcare Home Care company, is hiring a Full-Time RN Case Manager to provide one-on-one pediatric home health visits across Monroe, Lehighton, Northampton Counties.

This field-based, salaried role includes a company vehicle and comprehensive benefits. You will lead care in the home setting, assess patients, develop care plans, collaborate with physicians and families, and supervise LPNs and aides while ensuring regulatory compliance.

Qualifications

• Active PA RN license required.
• 1 year of nursing experience preferred.
• Pediatric or home health experience preferred.
• Valid driver’s license and CPR certification.

Responsibilities

• Complete comprehensive patient assessments, including OASIS documentation.
• Develop and oversee individualized pediatric care plans.
• Collaborate with physicians, families, and interdisciplinary care teams.
• Provide supervision and direction to LPNs and Home Health Aides.

This is a field‑based, salaried role with a company vehicle provided.

Schedule & Travel Expectations

• Full‑Time | 40 hours per week
• 5 days per week|Monday–Sunday | 40 hrs a week (Flexible schedule)
• Daily travel across assigned coverage counties based on patient needs

This role requires flexibility and comfort with travel within the listed counties.

What You’ll Do

Qualifications

• Pediatric or home health experience preferred
• Strong leadership and communication skills
• Ability to manage multiple priorities independently
• Valid driver’s license
• Current CPR certification
