CareGivers America – NEPA is seeking a full-time Pediatric RN Case Manager to coordinate care for pediatric patients and families across Luzerne, Wyoming, and Sullivan County. The role emphasizes meaningful 1:1 patient interactions and quality home-based care.

The position offers a Monday–Friday schedule, a company vehicle, and travel time within a 40-hour workweek, with no rotating weekends. Active PA RN license and CPR are required, with 1 year of nursing experience preferred.

Qualifications

• Active Pennsylvania RN license required.
• CPR Certification required or current.
• 1 year of professional nursing experience preferred.
• Valid driver’s license required.
• Pediatric or home care experience preferred.
• Strong leadership and communication skills.
• Ability to work independently in the field.

Responsibilities

• Complete patient assessments, including OASIS data collection.
• Develop and oversee pediatric care plans with physicians and families.
• Supervise and support LPNs and caregivers.
• Coordinate care with families and interdisciplinary teams.
• Ensure clinical documentation is accurate, timely, and compliant.
• Participate in quality improvement and staff education.
• Promote safe, high-quality care in the home.

Are you an RN who enjoys pediatric care but is looking for a more predictable schedule and meaningful one-on-one patient relationships?

CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Luzerne, Wyoming, and Sullivan County.

You’ll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of theirhome.

Your Role as a Pediatric RN Case Manager:

• Complete patient assessments, including OASIS data collection

What We're Looking For:
