Great Plains Tribal Health is seeking an RN Case Manager in Rapid City, SD. This role focuses on patient-centered care coordination within a Medical Home model, assessing and planning care for assigned patients and ensuring continuity across ED visits, hospital stays, and referrals.

The RN Case Manager collaborates with providers and community resources, and provides direct nursing care as needed while promoting safe, high-quality care and patient self-management.

Qualifications

• RN Case Manager duties include coordinating care across the Medical Home model.
• Requires RN license in good standing and BLS certification.
• Experience in case management and care coordination is preferred.

Responsabilités

• Coordinate care after ED visits, hospitalizations, and referrals.
• Develop and evaluate individualized care plans.
• Collaborate with providers and community resources for smooth transitions.
• Educate patients and families on condition management and self-management.
• Document care coordination and outcomes in the EHR.
