Job summary

Great Plains Tribal Leaders Health Board (GPTLHB) is seeking an RN Case Manager in Rapid City, SD to coordinate patient-centered care within a Medical Home model. You will assess, plan, implement, coordinate, monitor, and evaluate care for assigned patients, focusing on transitions after ED visits and hospitalizations.

The role requires RN licensure in good standing, BLS certification, and 3 years of RN experience (or 1 year RN + 4 years LPN).

Qualifications

• Graduate of a state-approved RN program (ADN, diploma, or higher).
• Current RN license in good standing.
• Three (3) years RN experience or one (1) year RN + four (4) years LPN.
• Current Basic Life Support (BLS) certification.

Responsibilities

• Coordinate patient-centered care within a Medical Home model.
• Assess, plan, implement, coordinate, monitor, and evaluate care for assigned patients.
• Manage care for patients after ED visits, hospitalizations, and referrals.
• Collaborate with providers and community resources to ensure continuity of care.
• Provide direct nursing care as needed across the lifespan.
• Document care coordination and outcomes in the EHR.
