Job summary

Lee's Summit Medical Center in Missouri is seeking a Registered Nurse Case Manager (PRN) to guide patients and families through recovery, coordinate services, and develop individualized care plans.

Collaborate with Rehab Program Director and the interdisciplinary team to ensure safe transitions, effective communication, and patient-centered care while meeting RN licensure and ACLS requirements.

Qualifications

• RN license required.
• ACLS within 60 days of employment start date.
• 1 year experience required.

Responsibilities

• Guide patients and families through program orientation, discharge criteria, and rights.
• Coordinate education for patients, families, and caregivers to support treatment planning.
• Develop and implement individualized treatment plans reflecting patient goals and needs.
• Complete psychosocial assessments and communicate findings to the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for seamless patient-centered care.
• Promote values of compassion, respect and excellence in interactions.
