Job summary

Qualifications

• RN licensure in Missouri required.
• Annual influenza vaccination required.

Responsibilities

• Develop a treatment plan responsive to patient/family concerns, age-specific problems and clinical impressions. Improves patient and/or family understanding of and adjustment to medical diagnosis through education.
• Concise documentation of needs identified, services provided, and discharge plan in patients’ medical records.
• Attend in-service, seminars, workshops, and training sessions related to Case Management to maintain and update clinical skills.
• Support hospital’s philosophy and mission. Formulates and maintains department policies and procedures.
• Maintain confidentiality and comply with professional/ethical codes, state and federal regulations, and TJC requirements.
• Facilitate multidisciplinary care plan meetings (rounding); recognize, monitor, and participate in discharge planning needs; work cooperatively with multidisciplinary teams and outside providers.
• Communicate with physicians, health plans, and other hospital staff regarding review outcomes, criteria, and various program requirements.
