Lee's Summit Medical Center in Missouri is seeking a Registered Nurse Case Manager (PRN) to guide patients and families through recovery, coordinate services, and develop individualized care plans.
Collaborate with Rehab Program Director and the interdisciplinary team to ensure safe transitions, effective communication, and patient-centered care while meeting RN licensure and ACLS requirements.

As a Case Manager, your rolewill be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs.
