The role requires the candidate to be located in Alaska or Washington and focuses on facilitating patient access to quality, cost-effective care, developing patient-centered goals, and providing reports on findings and outcomes.

Competenze

• RN license required.
• 4 years of clinical experience.
• URAC credential standards.
• Preferred: Motivational Interviewing experience.

Mansioni

• Screen and assess case management patients.
• Develop a patient-centered care plan with short and long-term goals.
• Plan and complete interventions and service coordination related to the care plan.
• Provide patient and family advocacy during the case management process.
• Supervisory input on non-clinical staff performance when applicable.
