Job summary

Comagine Health in Seattle, WA, is seeking an RN case manager to facilitate patient access to quality, cost-effective care and to develop patient-centered care plans in collaboration with the care team.

The role requires an active RN license, 4+ years clinical experience, and URAC credential standards; CCM certification and motivational interviewing are preferred. You will mentor staff and advocate for patients and families throughout the care process.

Purpose of Position:

• Facilitate patient access to quality, cost-effective health care.
• Develop and monitor patient-specific goals and interventions in collaboration with attending physician and health care team.
• Provide reports on findings, recommendations, and outcomes.

Typical Duties:

• Screen and assess case management patients.
• Develop a specific patient centered care plan, complete with short and long-term goals.
• Plan and complete interventions, service coordination and implementation, related to the care plan.
• Provide patient and family advocacy during the case management process.
• As appropriate, provide daily oversight and monitoring of non-clinical staff during their performance of nonclinical support activities; also provide the supervisor with input regarding employees' performance of these activities.

Possess current and active licensure or certification in a health or human services discipline with a legal scope of practice that permits independent assessment.
