Comagine Health in Seattle, WA, is seeking an RN case manager to facilitate patient access to quality, cost-effective care and to develop patient-centered care plans in collaboration with the care team.

The role requires an active RN license, 4+ years clinical experience, and URAC credential standards; CCM certification and motivational interviewing are preferred. You will mentor staff and advocate for patients and families throughout the care process.

Competenze

• RN licensure with active status.
• 4 years of clinical experience.
• URAC Credential Standards applied.
• Preferred: CCM certification and motivational interviewing experience.

Mansioni

• Screen and assess case management patients.
• Develop patient-centered care plans with short and long-term goals.
• Plan interventions and coordinate services per the care plan.
• Provide patient and family advocacy during the case management process.
• Oversee non-clinical staff performing support activities and provide performance input to supervisor.
