Job summary

Comagine Health is a national, nonprofit health care consulting firm dedicated to improving health care delivery. This RN case management role focuses on facilitating patient access to quality, cost-effective care and developing patient-centered care plans in collaboration with the care team.

Responsibilities include screening and assessing patients, planning interventions, coordinating services, and advocating for patients and families throughout the process.

Qualifications

• RN license must be current and in good standing.
• Minimum 4 years of direct clinical experience in health care.
• URAC Credential Standards compliance preferred.
• CCM certification preferred.

Responsibilities

• Screen and assess case management patients.
• Develop a patient-centered care plan with short and long-term goals.
• Plan and coordinate interventions and services to implement the care plan.
• Provide patient and family advocacy during the case management process.
• Oversee non-clinical staff performance when appropriate and provide supervisor input.

This position requires the person to be located in Alaska or Washington.

Purpose of Position:

• Facilitate patient access to quality, cost-effective health care.
• Develop and monitor patient-specific goals and interventions in collaboration with attending physician and health care team.
• Provide reports on findings, recommendations, and outcomes.

Typical Duties:

• Screen and assess case management patients.
• Develop a specific patient centered care plan, complete with short and long-term goals.
• Plan and complete interventions, service coordination and implementation, related to the care plan.
• Provide patient and family advocacy during the case management process.
• As appropriate, provide daily oversight and monitoring of non-clinical staff during their performance of nonclinical support activities; also provide the supervisor with input regarding employees' performance of these activities.

Requirements

Required

• RN
• 4 years clinical experience
• URAC - Credential Standards
• Preferred experience with motivational interviewing.
• Possess current and active licensure or certification in a health or human services discipline with a legal scope of practice that permits independent assessment.

Preferred

• 2 years CM
• CCM certification
