You will develop patient-centered care plans, monitor progress, and provide advocacy for patients and families. The role emphasizes clinical leadership, coordination of services, and adherence to credential standards.

Competenze

• RN licensure and 4 years of clinical experience required.
• URAC credential standards familiarity.
• CCM certification preferred.
• Active licensure in a health or human services discipline.

Mansioni

• Screen and assess case management patients.
• Develop patient-centered care plans with goals.
• Plan and execute interventions and service coordination.
• Provide patient and family advocacy during care.
• Oversee non-clinical staff performing support activities.
