Qualifications

• Bachelor's or RN diploma or associate degree required.
• Registered Nurse (RN) license required.
• No travel required.

Responsibilities

• Guide patients and families through orientation, discharge criteria, and rights.
• Coordinate education for patients, families, and caregivers for treatment planning.
• Develop and implement individualized treatment plans reflecting patient goals.
• Complete psychosocial assessments and communicate needs across the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for seamless care.
• Promote compassion, respect, and excellence in every interaction.
