Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered RN license.
• 1+ years of case management experience, preferably in healthcare.
• Strong interpersonal and communication skills with medical teams, patients, and families.
• Proficiency in electronic health records (EHR) software.

Responsibilities

• Assess patients' medical history, medications, and psychosocial needs.
• Collaborate with providers, patients, and families to create care plans.
• Monitor progress and adjust plans; communicate changes to the team.
• Serve as main contact between patient and providers; maintain records.
