Job summary

Qualifications

• BSN or higher degree from an accredited program.
• Active, unencumbered RN license.
• 1+ year experience in case management.
• Strong interpersonal and written/verbal communication.
• Proficient with electronic health records (EHR) software.

Responsibilities

• Perform comprehensive patient assessments including medical history and psychosocial needs.
• Develop and implement individualized care plans with care team.
• Monitor patient progress and adjust plans as needed.
• Act as main point of contact coordinating between patient and providers.
• Maintain confidential, compliant case notes and records.
