Job summary

HCA Houston Healthcare Northwest is seeking a Registered Nurse Case Manager to guide patients and families through care transitions and coordinate resources across the care continuum. Qualifications

• RN license required.
• Bachelor's Degree or Associate Degree in a relevant field.

Responsibilities

• Guiding patients and families through program orientation, explaining rehabilitation philosophy and discharge criteria.
• Coordinating education for patients, families, and caregivers to participate in treatment planning and family conferences.
• Developing and implementing individualized treatment plans reflecting patient strengths and goals.
• Completing psychosocial assessments and discharge planning evaluations and communicating needs across the care team.
• Collaborating with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team for seamless, patient-centered care.
• Promoting HCA Healthcare values of compassion, respect, and excellence in every interaction.
