Qualifications

• BSN or higher from an accredited program.
• Active RN license.
• 1+ year experience in case management.
• Strong written and verbal communication to collaborate with clinicians, patients and families.
• Proficiency with electronic health records (EHR).

Responsibilities

• Conduct comprehensive assessments of patient history, medications, and psychosocial needs.
• Collaborate with healthcare providers to develop and implement individualized care plans.
• Monitor progress and adjust plans as needed with clear communication to the team.
• Serve as primary liaison between patient and providers to coordinate care.
• Maintain up-to-date records and ensure confidentiality and compliance.
